Your ears can hurt when you fly because of an imbalance in pressure between your middle ear and the cabin of the airplane. Usually pressure in your middle ear is equalized by your eustachian tube. But if that tube gets clogged due to congestion or some other problem, the pressure in your eardrum becomes unbalanced causing mild hearing loss, ringing or pain. If your eustachian tube becomes completely blocked, changes in pressure could be so severe that your middle ear may develop fluid build-up or even start to bleed.
